Eye Report Form D502
A first time application for a learner permit must be accompanied by a satisfactory eyesight report.
This report may be completed by a registered ophthalmic optician or by a registered medical practitioner.
The applicant must sign the eyesight report in the presence of the optician or doctor.
Applicants are not required to submit an eyesight report with each subsequent learner permit or driving licence application.
However, please note that if an applicant previously needed to wear corrective lenses for driving purposes but no longer need to
do so they must submit a new eyesight report stating that they no longer have to wear corrective lenses for driving purposes.
This may occur if an applicant has recently had laser or corrective surgery.
