Medical Report - Form D501
An application for a learner permit or driving licence must be accompanied by a medical report if:
- You are applying for a driving licence or learner permit in respect of any of the categories C1, C, D1, D, EC1, EC, ED1 or ED
unless you have previously provided a medical report which is still applicable
- You will be 70 years of age or more on the first day of the period for which the licence is being issued
- You suffer from any of the following disabilities or diseases
- Alcoholism
- Any physical disablement which is likely to affect ability to drive safely
- Any illness which requires the regular use of psychotropic substances
- Any illness or disease which requires the regular use of medications likely to affect the ability to drive safely
- Cardiovascular diseases, diabetes, serious diseases of the blood.
- Diplopia, defective binocular vision or loss of visual field
- Encephalitis, multiple sclerosis, myasthenia gravis or hereditary diseases of the nervous system
associated with progressive muscular atrophy and congenital myotonic disorders
- Epilepsy, diseases of the peripheral nervous system, trauma of the central or peripheral nervous system
- Cerebrovascular diseases, lesion with damage to spinal cord and resultant paraplegia
- Mental disturbance due to disease or trauma of, or operation upon, the central nervous system
- Severe mental retardation, Psychosis, Psychoneurosis or personality disorders
- Serious hearing deficiency
- Serious diseases of the blood
- Any disease of the genito-urinary system (including renal disorder) which is likely to affect ability to drive safely
- You have ever suffered from alcoholism or epilepsy
- • You are taking, on a regular basis, drugs or medicaments which would be likely to cause the
driving of a vehicle by you to be a source of danger
The applicant must sign the medical report in the presence of the medical practitioner.
Note:
- A person who suffers from serious arrhythmia, which has at any stage resulted in loss of consciousness, is
advised to consult his/her doctor before applying for a licence.
- A person who is dependent on or regularly abuses psychotropic substances is disqualified from holding any
learner permit or driving licence.

Medical requirement for licences in the higher categories
As a result of Road Traffic (Licensing of Drivers) Regulations, 1999, which came into effect on 15th November, 1999,
all holders of full driving licences for categories C1 (large van/small truck), C (rigid truck), D1 (minibus), D (bus), EC1
(large van/small truck with trailer), EC (articulated truck), ED1 (minibus with trailer) or ED (bus with trailer) must now
produce a Medical Report (completed by a doctor) to retain these categories on their licences. If an applicant has previously
submitted an unlimited medical report for these categories it is deemed to be valid for ten years from the date of issue
If the applicant wishes to keep any of the above categories on their licence they must now submit a Medical Report (D501) when
renewing their licence. However, if they no longer wish to have these categories on their licence they may complete a declaration
stating this. Their licence will then be issued for categories B (car), EB (car & trailer), W (tractor) and categories A1 and A
(motorcycles) where applicable.
It should be noted that entitlement to the higher categories on a licence or permit expire when the medical expires.
If the applicant does not submit a medical to renew any higher categories held, they will not receive this categories and will
automatically be issued with licence for which ever of the following categories they previously held A1, A, B, EB, W.
