County Wicklow Age Friendly Strategy 2023-27

Action 3.8 Appoint Healthy Age Friendly Homes Co-ordinator & Rollout Wicklow Programme 

 

Healthy Age Friendly Homes (HAFH) Programme is a support coordination programme that aims to enable older people to age in place and reduce the need to transfer to long-term residential care. The Programme is a collaboration between the Department of Health and the Department of Housing and Local Government. It is funded by the Department of Health and operated by Meath County Council, on behalf of all Local Authorities. 

 

The service is designed to enable older people to remain living in their own home for longer. The local coordinator undertakes home visits to older people living in the community, where a needs assessment is carried out, looking at four key areas of support: housing, health, community/social support, and technology. Home visits from the local co-coordinator are available to anyone aged 65+ years. The rollout of the programme commenced in January 2024.

Older People’s Council, Executive Group - Meetings and Representation 

The Older People's Council (OPC) Executive Group had a meeting in the Brockagh Centre on Thursday 18th April, 2024 where they welcomed Wicklow`s New Age Friendly Programme.
